Students should write their name, father’s name and mother’s name as per the mark sheet. Otherwise, if the form is rejected, it will be the responsibility of the student himself.
Note:-
- SIS registration is mandatory for online students.
- Students who have not yet registered in SIS should register because after the process of migration certificate is completed, you will be able to download the migration certificate from SIS login only.
- Students who were admitted offline before 2014 will get the migration certificate offline from the university.

The facility of filling online form and paying fee has been made available directly to the students. Hence, students should fill the form themselves and pay the fee. The students themselves will be responsible for payment made from any other person or internet cafe.
This online form is valid only when its payment has been made online, in which details of Application number, QR code, Transaction Id, Payment date, Channel Id etc. are clearly mentioned, otherwise the form is considered cancelled.
