1. Requisition Letter addressed to The Registrar, The Tamilnadu Dr. M.G.R. Medical University, Guindy, Chennai 600 032.
- Kindly Mention College Name, Course Name and Register Number in requisition letter. If the applicant is presently a student of Affiliation Institution, then the requisition letter should be forwarded by the Head of the Institution studying. A scanned copy of the this letter upload in online.
2. Scanned copy of SSLC / HSC Mark statement
3. Scanned copy of concerned certificate (E.g: Marksheet, Provisional Certificate)
4. Hard copy of the Original concerned certificates to be sent by Post or Submitted in-person to the University along with the acknowledge mail copy received from the University while applying online.
Fees : Rs.1000/- per year of delay each certificate
