1. Requisition Letter addressed to The Registrar, The Tamilnadu Dr. M.G.R. Medical University, Guindy, Chennai 600 032.   

  1. Kindly Mention College Name, Course Name and Register Number in requisition letter. If the applicant is presently a student of Affiliation Institution, then the requisition letter should be forwarded by the Head of the Institution studying. A scanned copy of the this letter upload in online.       

2. Scanned copy of SSLC / HSC Mark statement

3. Scanned copy of concerned certificate  (E.g: Marksheet, Provisional Certificate)

4. Hard copy of the Original concerned certificates to be sent by Post or Submitted in-person to the University along with the acknowledge mail copy received from the University while applying online.

Fees : Rs.1000/- per year of delay each certificate  

Cover & Postal Charges: 

Postal Cover: Rs. 50/-

(a) Rs. 50/- for sending within India.

(b) Rs. 1400/- for sending to Canada.

(c) Rs. 2150/- for sending to USA.   

(d) Rs. 2320/- for sending to UK.

(e) Rs. 1900/ for other countries.
