1. Requisition Letter addressed to The Registrar, The Tamilnadu Dr. M.G.R. Medical University, Guindy, Chennai – 600 032.
Note: Mention College Name, Course Name, Register Number and Postal address to which the certificate needs to be sent in the requisition letter.
2. An affidavit on Rs.200/- non-judicial stamp paper from the Notary Public after the date of issue of Non Traceable Certificate (Click here to download the prescribed affidavit)
Original Affidavit to be sent by Post / Courier or Submitted in-person to the University along with the acknowledge mail copy received from the University while applying online.
3. Scanned copy of HSC Mark Statement .
4. A Bonafide Certificate from the concerned Institution affixing latest passport size photo with candidate’s signature duly attested by the Principal of the Institution.
5. Latest passport size photo (<20kb size)
6. Scanned copy of Provisional Certificate
7. Non Traceable Certificate / Lost Document report.
